When a tooth bud doesn’t fully erupt from the bone or gum — dentists call it tooth retention. The most common is wisdom teeth retention, less often canines and incisors. In some cases, the condition doesn’t cause pain, but inflammation, swelling, and pain may be present.

An impacted tooth is a tooth that has not erupted or only partially protrudes above the gum surface. Retention of teeth is a rather painful condition that often disrupts daily life. The reasons why a tooth does not properly emerge are very diverse – from genetic conditions to the late loss of milk teeth.
Impacted Tooth – Symptoms
The retention of front teeth is easy to detect. It’s worse when the problem concerns molars, particularly wisdom teeth. Not everyone has a rudiment of a wisdom tooth, so its absence might go unnoticed. The most common symptoms of tooth retention include:
- Inflammation of the gums above the unerupted tooth
- Incorrect bite, pain when widely opening the mouth
- Chronic pain caused by the tooth pressing against a nerve buried deep in the lower jaw
- Periodontitis of neighboring teeth
- Cavities in the erupted part of impacted teeth
- Frequent infections
- Root of the tooth being pushed into the surrounding tooth causing root fracture or fracture of the entire neighboring tooth
Retention Diagnosis
An examination might not be sufficient; therefore, to recognize an impacted tooth, an x-ray – specifically an orthopantomogram – is required. After obtaining the image, the dentist can determine further treatment.
Wisdom Tooth Retention
Wisdom teeth create numerous problems not only for their owners but also for dentists because access to them is often very challenging. Upper wisdom teeth are generally much easier to remove than lower ones. Surgical removal of lower wisdom teeth usually involves cutting the gum behind the seventh tooth to access the bone where the tooth is hidden. Usually, the doctor prescribes painkillers (sometimes very strong ones) and antibiotics to control bacteria that may enter the bloodstream from the affected tooth.
Wisdom tooth retention is a common complication of tooth eruption characterized by a lack of space in the retromolar area, incorrect placement of the wisdom tooth relative to the distal section of the alveolar part of the jaw and its branch, as well as to another molar.
Treatment of Tooth Retention
Treatment of poorly growing teeth is carried out by installing an individually selected orthodontic appliance. Proper adjustment of the device moves the tooth into the correct position. Orthodontic treatment should begin as early as possible when the teeth are not yet firmly within the jaw.
In deciding on such treatment, be patient, as it may take up to two years to bring the tooth back to the desired spot. But persistence is worth it, as a proper bite and a beautiful smile are not only questions of aesthetics but our health.
